One of New York’s finest skilled nursing centers, The Osborn Pavilion is available to The Osborn residents on a priority admission basis, as well as those who need long-term care in Rye, New York, and surrounding Westchester County communities.

Our mission is to offer the finest services, amenities, and professional care anywhere. Everything about this exceptional health care center has been designed with the residents’ comfort and well-being in mind.

All resident rooms at The Osborn Pavilion are private with their own bathroom. The comprehensive medical suite includes exam rooms, and offers primary care, cardiology and podiatry services through an on-campus satellite office of WESTMED, the Westchester Medical Group. The Osborn is also just steps away from WESTMED’s newest multi-practice facility, offering pharmacy and urgent care services. If necessary, referrals are made to the group’s main offices in Rye for any diagnostic or specialty services. In addition to the services provided by WESTMED, ophthalmology and dentistry services are available on-site through our contracts with two independent providers.

The Osborn Pavilion is Medicare-certified. Health care services are charged on a per diem basis. These may be covered by long-term care insurance, qualified HMOs, qualified Medicare and/or private payment.

Skilled Nursing in a Home-like Atmosphere

The Osborn Pavilion features 84 private resident rooms, clustered in groups of seven or eight. This design approach helps create a warm sense of neighborhood for residents and their guests. Each spacious, private room is furnished and decorated to create a comfortable, home-like setting. Just like our independent senior living residents, individuals here are encouraged to bring their own touches such as pictures and mementos to make their rooms comfortable and familiar.

Along with the comfort of expert skilled nursing, numerous activities and amenities are available for residents’ enjoyment. An expert dining services staff, under the direction of our Culinary Institute of America-trained executive chef, prepares nutritious restaurant-style menu selections served in one of two elegant dining rooms or the resident’s own room, as preferred.

The Osborn Pavilion’s commitment to providing the finest long-term care begins with our staff of dedicated professionals who ensure each resident receives care tailored to their individual needs and preferences. The Osborn Pavilion provides 24-hour skilled nursing care and these specialized services:

Short-Term Rehabilitation (Sub-Acute Care)
The Pavilion offers short-term rehabilitation, outpatient rehabilitation and restorative treatment to individuals following a fracture, orthopedic surgery or a neurological event. The program is designed to benefit those who desire a highly individualized program of integrated therapy and restorative nursing services, provided in a state-of-the-art skilled nursing center.

Respite Care
Placing a loved one for a short “respite” in the comfort and security of The Osborn Pavilion provides relief to family caregivers.

Dementia and Alzheimer’s Care
When someone you love has advancing memory impairment, Alzheimer’s disease or another form of dementia, you’ll realize the time may come when they can no longer life safely at home, or perhaps that time is imminent. In either case, we invite you to learn more about our long-term care programs for memory loss. Here residents live in private rooms in our handsomely decorated skilled nursing center and are encouraged to bring favorite mementos from home. At The Osborn Pavilion, your loved one’s dignity will be maintained, their uniqueness will be celebrated and their quality of life will be at the highest level possible.

On The Osborn campus, memory care residents enjoy a beautifully landscaped, protected Wander Garden and attend concerts, movies, events and other programs with their caregivers as needed. Residents receive 24-hour dementia care services from our on-site medical director, dementia care specialist and nursing staff who develop a custom memory care plan to suit each resident’s individual interests, align with their abilities, and promote their sense of well-being and self-respect.

Hospice Care
A specially trained staff from Hospice of Westchester works with Osborn Pavilion staff to meet the needs of the terminally ill and their families, including pain management.
