The Osborn's in-home care staff members receive extensive training and certification beyond state requirements. They approach each case with a genuine desire to help people who choose to stay in their own homes, but need a little extra care and support.

Offer services tailored to each client's specific needs, Osborn Home Care provides a thorough evaluation and professional assessment in consultation with the individual, the family and the primary physician. A Nurse Case Manager develops and implements a Home Care Plan that addresses personal values and preferences, as well as health care needs. Professional case management services include monitoring and coordinating all aspects of a client's care, performing regular home health assessments, scheduling physician visits and special care services, medication assistance and 24/7 on-call RN assistance.

A Professional, Highly Trained Staff

Professional senior home care services are provided by Registered Nurses and Case Managers. Supportive services are provided by trained, professional Certified Home Health Aides under the supervision of a Registered Nurse. Osborn Home Health Aides are valued employees of The Osborn, carefully screened, trained and supervised to ensure they provide the highest quality care and customer satisfaction. In addition to certification training, our Aides receive ongoing training in geriatric care and how to handle clients with special needs due to dementia.

Licensed by the New York State Department of Health and a member of the New York State Association of Health Care Providers, Inc., Osborn Home Care accepts private pay and most insurance policies for senior in-home care.

Financial assistance to pay for these services is available through the Osborn Home Care Scholarship Program, for those who qualify.
